Register Template for Tracking All Hazards, Risks, Controls and Risk Assessments Completed


A simple register that can be used to identify and review hazards and risks.

To assist with hazard review and analysis a planning Hazard-Review sheet includes a listing of typical hazards such as 'Fall from height' with a corresponding hazard description and then a select box to enter whether this hazard is applicable and if it is the risk rating and further details about the hazard. 

The main listing is the Activities-Hazards sheet where an overview of hazards can be summarised for each area or process including details of the hazards and persons at risk followed by an inherent risk analysis which makes use of select boxes for 'Likelihood' and 'Consequence' to automatically determine the Risk Rating using the Risk Level Indicator. A summary of control measures, residual risk rating and also a review of relevant legislation and compliance can all be detailed along with notes about past incidents and other relevant comments.

This file makes use of conditional-formatting so higher level risks are clearly displayed.

The register also includes a listing of all Risk-Assessments with details of frequency and date fields using conditional formatting to provide a prompt when risk assessments are due to be renewed or overdue.

This register can be used to cover all risks including some of the lower level risks where the register itself can serve as the documented risk assessment.
For areas where further risk assessments are required the register can be used to detail what is planned, dates due and who is responsible.
If completed correctly this register will ensure all the risk related considerations covered by ISO 45001 are covered.
